Office Hours: Monday – Friday, 9a-6p

A Transitions of Care Appointments is a follow-up visit after leaving the emergency room or after a hospital stay, that acts as a bridge between the hospital visit and the patient’s long-term care plan.
Providers will create a personalized care plan for patients that can include:
- Medication Management
- Follow-up Appointments
- Specialist Referrals
- Education on Disease Management
- Lifestyle Changes
Required Qualifications
- Board Certified FNP or PA with active Texas license and TX DEA.
- Preferably with a background in Emergency Medicine, Urgent Care or Primary Care.
